Sex hormone-binding globulin (SHBG) is a liver-made protein that binds testosterone and estradiol in the bloodstream. Only the unbound fraction is biologically active. A man can have a "normal" total testosterone and still feel every symptom of low testosterone if his SHBG is high. That is the case standard panels miss most often.

Never interpret total testosterone alone. Total testosterone plus SHBG plus albumin — used to calculate free testosterone — is the minimum panel for a real read on hormonal status.

What SHBG physically measures

Testosterone travels in the blood in three states. Roughly 40–60% is bound tightly to SHBG and cannot leave circulation. Most of the remainder is bound loosely to albumin and can dissociate at the tissue. Only about 1–3% circulates completely free.

Total testosterone adds all three together. SHBG tells you how that total is distributed. Two men with identical total testosterone and SHBG values of 20 and 60 nmol/L have meaningfully different amounts of hormone reaching tissue.

Why the number matters in both directions

High SHBG creates a hormonal problem: plenty of testosterone in the tank, not enough reaching cells. Low SHBG usually signals a metabolic problem, because insulin suppresses SHBG production in the liver. The two abnormal directions have almost nothing in common clinically.

That second point is underappreciated. Low SHBG is one of the stronger single predictors of developing type 2 diabetes, and it often appears before fasting glucose moves. A 30-year-old man with SHBG of 14 nmol/L and no medication history has a metabolic finding on his hormone panel.

There is no drug that targets SHBG directly, and there should not be — SHBG is a readout, not a target. Raising a low SHBG means treating insulin resistance, fatty liver, or thyroid function. Lowering a high SHBG means addressing an energy deficit, a thyroid excess, or a medication, where one of those is the cause.

How and when to test

  • Draw between 7 and 10 a.m. Testosterone follows a strong circadian rhythm, and free testosterone is calculated from it. An afternoon draw can read 20–30% lower in younger men.
  • Fast beforehand. A meal, particularly a glucose load, acutely lowers testosterone.
  • Stop high-dose biotin for 72 hours. See the callout above.
  • Order total testosterone, SHBG, and albumin together. All three are needed to calculate free testosterone.
  • Confirm any abnormal result with a second morning draw before acting. Single-sample hormone diagnosis is unreliable.
  • Do not test during acute illness or within a week of a major training block. Both suppress the axis temporarily.
  • Note oral contraceptive or oral estrogen use on the request. Standard ranges do not apply, and after stopping, SHBG can take several months to return to baseline.

What it means in combination

  • High SHBG + normal total testosterone + symptoms: free testosterone is the actual problem. Treatment may still be appropriate despite a normal total.
  • Low SHBG + normal or high total testosterone: usually insulin resistance. Check fasting insulin, HOMA-IR, triglycerides, and ALT before touching hormones.
  • Low SHBG + high free androgens in a woman: the classic PCOS pattern, and the reason free testosterone can be high while total looks unremarkable.
  • High SHBG + suppressed TSH: think hyperthyroidism first. Treating it often resolves the hormone picture without any other intervention.
  • Falling SHBG on testosterone therapy: expected. It means calculated free testosterone rises faster than total, so dosing decisions should track free, not total.
  • High SHBG + low total testosterone + high LH: points to primary testicular failure rather than a binding-protein issue, and needs an endocrine workup.

  • Persistent symptoms — low libido, fatigue, mood change, loss of morning erections — with any SHBG value, since symptoms outrank the number.
