Why the Zuclomiphene Fraction Gets Blamed
Zuclomiphene behaves as a weak estrogen agonist, and it leaves the body slowly. Enclomiphene clears within about a day. Zuclomiphene is measured in days to weeks, so daily dosing means the concentration climbs for a month or more before it settles.
That accumulation is the standing explanation for why some men on clomiphene report low mood, flat libido, or a foggy feeling that does not match their improving testosterone number. It is a plausible mechanism. It is also, at this point, a hypothesis: no published trial has isolated zuclomiphene and shown it causes those symptoms in men.
Repros Therapeutics built its entire development programme on the theory. If the accumulating isomer is the problem, a drug without it should work as well with fewer complaints. That was the case put to regulators, and it is still the case telehealth platforms make on their landing pages. It has not yet been proven in a trial that gave one group clomiphene and the other enclomiphene.

The two evidence bases have opposite shapes. Clomiphene in men rests mostly on observational cohorts and single-arm follow-up, some running for years, showing testosterone moving from clearly low into the normal range and symptoms improving with it. The design is weak and the duration is long.
Enclomiphene has the better designs and the shorter horizon. Its studies were randomised and placebo-controlled, several with an active testosterone gel comparator, and they consistently showed testosterone rising while LH and FSH rose too. They also stopped at 16 weeks. What happens at two years is documented for clomiphene and unknown for enclomiphene.
The Approval Gap Runs Backwards
Buyers usually assume the newer, more refined drug is the more regulated one. Here it is the reverse, and the practical consequences are the part worth understanding.
Clomiphene citrate holds FDA approval for ovulation induction in women. Prescribing it to a man is off-label, which is lawful and routine. A clinician can still open an approved label that lists warnings, contraindications, interactions, and what to do when visual symptoms appear. Any pharmacy dispenses it. It is a generic, so it costs very little.
Enclomiphene has no approved product anywhere. Repros Therapeutics ran it through Phase III as Androxal and the FDA issued a Complete Response Letter in 2015. Nothing has been approved since. Every enclomiphene product on sale is compounded, prepared by a pharmacy against a prescription, and compounded preparations sit outside the FDA approval process entirely. They are not reviewed for safety, efficacy, or quality, there is no approved insert to check an interaction against, and potency depends on the individual pharmacy.

Clomid fits the situation better when any of these apply:
- Cost matters, and a generic prescription is the difference between treating and not treating.
- A clinician wants approved labelling to prescribe and monitor against.
- The plan is a long course, where the multi-year record in men counts for more than a 16-week trial.
- No compounding pharmacy is available or affordable where you live.
Enclomiphene fits better when a previous course of clomiphene produced mood or libido complaints that improved on stopping, and a clinician judges the zuclomiphene explanation worth acting on.
Who this comparison is not for: men with primary hypogonadism, where high LH and FSH mean neither drug has a mechanism to use. Also men whose low testosterone is being driven by something reversible. Obesity, untreated sleep apnoea, opioid use, and pituitary lesions all suppress testosterone, and several of them respond to treating the cause. Reading signs of low testosterone alongside a workup is a better starting point than a drug comparison.
What would change our answer: a randomised head-to-head trial of clomiphene against enclomiphene in hypogonadal men, reporting mood and libido as endpoints rather than testosterone alone. If that trial found enclomiphene meaningfully better tolerated, the approval gap would stop being the deciding factor. An FDA approval for an enclomiphene product would do the same thing faster.

That gap is itself worth knowing about. It means the follow-up interval on a compounded enclomiphene programme is set by the platform selling it, not by an external standard.
