The published human trials of NMN report almost no side effects. In the guides we publish here, that sentence is where most NMN safety pages stop, and stopping there hands the reader a reassurance the evidence does not support.
Every one of those trials ran for 60 days or less, in groups of 10 to 80 people. A supplement bought by people who intend to take it daily for decades has been tested for a period roughly the length of a summer holiday. What the trials establish is short-term tolerability at the doses studied. What they cannot establish is what a decade of daily use does.

Four trials carry most of the safety discussion in this category, and they agree with each other. None reported a serious adverse event attributable to NMN.

The mild effects that do appear are nausea, headache, flushing and loose stools, at rates that generally did not separate from placebo. Taking a capsule on an empty stomach accounts for a share of the nausea reports.
Read the fourth column of each row before the third. The reassurance in these trials is real and it is narrow, and the narrowness is the part that determines what it means for someone planning to take NMN for years.
The Doses Those Safety Findings Cover
The studied range runs from 100 mg as a single dose up to 1,200 mg daily for six weeks. Most commercial products sit at 250 to 1,000 mg per serving, which places typical consumer use inside the range that has been examined.
Above roughly 1,200 mg there is no human data. That is not a statement that higher doses are harmful. It is a statement that nobody has looked, which is the more useful thing to know when a product page recommends escalating.

Where the Safety Data Runs Out
Three gaps sit between the trial evidence and how NMN is used.
- Duration. Sixty days at most, against intended use measured in years. Effects that accumulate slowly would not appear.
- Sample size. Groups of 10 to 80 people can detect a common side effect and cannot detect a rare serious one. An event occurring in one user per thousand would be invisible across every NMN trial combined.
- Who was studied. Participants were largely healthy and medication-free. People with liver disease, kidney disease, active cancer or a long prescription list were excluded, and those are exactly the people for whom a safety question is most pressing.
None of this is unusual for a supplement, and it is worth stating plainly because the NMN category markets itself on scientific rigour more than most.
The Theoretical Concern Worth Taking Seriously
One mechanistic concern deserves more weight than the general "we do not know" caveat, and it concerns cancer.
NAD+ supports cellular energy metabolism, and rapidly dividing tumour cells have high NAD+ demand. Pharmaceutical companies have developed NAMPT inhibitors, which block NAD+ synthesis, as experimental anticancer agents. That is the same pathway NMN supplementation is designed to push in the opposite direction.
Whether this matters clinically in a person taking an oral supplement is unresolved, and the animal literature points in both directions. The reasonable position is that anyone with an active cancer diagnosis or under surveillance for one should raise NMN with their oncology team rather than deciding from a supplement label. Purity Is a Bigger Practical Risk Than Toxicity
For most buyers, the realistic risk is not what NMN does. It is whether the capsule contains what the label says.
NMN went through three years of regulatory exclusion by the US Food and Drug Administration between late 2022 and September 2025, during which supply chains shifted and major retailers delisted products. A brand that changed material sources during that period is not automatically covered by anyone's new dietary ingredient notification. Independent testing of the category has repeatedly found products whose measured NMN content diverges from the label.
Two checks are worth making before buying. Look for a batch-specific certificate of analysis from an independent laboratory rather than a generic purity claim, and check whether the brand names its ingredient supplier. Drug Interactions and Who Should Avoid It
No specific drug interaction is established for NMN, and that reflects thin data rather than a clean bill. Trial participants were largely medication-free.
Four groups should not start NMN without asking a clinician first.
- Anyone with an active cancer diagnosis or in surveillance. The NAD+ mechanism runs in the opposite direction to an experimental drug class, and the question belongs with the oncology team.
- Anyone pregnant or breastfeeding. No trials have included this group, so there is no data either way.
- Anyone with liver or kidney disease. Excluded from the trials, so the safety findings do not extend to them.
- Anyone on a narrow therapeutic index medication such as warfarin, lithium or an antiepileptic. NAD+ is a cofactor across a wide set of enzyme reactions, and a pharmacist can check the specific drug.
Our reading would change if a trial ran NMN for two years or longer with a placebo arm and routine safety monitoring. That is the study the category needs and has not produced, and its absence is the real limit on every safety claim made for NMN, including the reassuring ones. A two-year trial reporting nothing would move this from unstudied to reasonably established.

What dose of NMN causes side effects?
